Cancer Survivorship Care
Life After Cancer: A New Chapter in Your Health
Cancer survivorship begins the moment treatment ends, but the journey does not stop there. At Willow Health in Fargo, ND, we believe survivorship care is just as important as treatment itself. Cancer survivorship is one of the areas Willow Health specializes in, with care built around the treatments you received and the long-term effects they may cause. Our goal is to support you physically, emotionally, and socially so you can thrive in survivorship.
Components of Survivorship Care
Personalized Survivorship Plans
Every patient receives a detailed treatment summary that records diagnosis details, staging, pathology, and all chemotherapy, radiation, and surgical treatments. This plan guides your long-term care. We follow guidelines from the National Comprehensive Cancer Network (NCCN) to ensure your care remains evidence-based and tailored to your treatment history.
Ongoing Monitoring and Follow-Up
Survivorship care focuses on the long-term effects of treatment, not just the type of cancer. Depending on your history, your plan may include:
- Regular blood work
- Imaging when indicated
- Heart or lung function testing
- Bone density screenings
- Hormone or metabolic evaluations
You will know what to expect, how often tests are needed, and what symptoms to watch for.
Managing Long-Term Side Effects
Survivorship often comes with new health challenges. We help you manage:
- Fatigue and sleep changes
- Heart and lung effects from treatment
- Hormone shifts and bone health
- Neuropathy or nerve pain
- Emotional health, including anxiety or depression
Our goal is to anticipate potential side effects based on your treatment record and help you manage them with proactive care.

How will my primary care doctor know what to do?
Your Willow Health provider uses your treatment summary and NCCN guidelines to create an individualized plan, then coordinates with your oncology team as needed.
What follow-up tests or monitoring do I need?
Potential effects may include fatigue, heart or lung changes, hormone shifts, bone density loss, or anxiety. We help you anticipate and manage these concerns with personalized care and supportive resources.
What long-term side effects or risks might I face?
Long-term effects depend on the treatments you received. Chemotherapy, radiation, or hormone therapy can impact heart, lung, nerve, or bone health and may cause fatigue or emotional changes. At Willow Health, we monitor for these effects through tailored follow-up care and screenings to help you stay healthy and active long after treatment ends.

We work closely with local and state organizations to ensure survivors get the resources they need.
Emotional and Mental Health Care
We recognize that survivorship is more than physical healing. Our providers support your emotional health and connect you with counseling, group resources, and stress management strategies to help you feel whole again.
